Why Virtual Cases? A Note on Patient Privacy

Every doctor in India is bound by the National Medical Commission's Code of Ethics regarding patient confidentiality — and since 2025, by India's Digital Personal Data Protection (DPDP) Act, 2023, now in force under a phased rollout. Together, these mean a trainee's personal logbook or self-practice tool simply cannot legally contain identifiable patient information, however anonymised it might seem.

CCEF's OSCE Case Generators exist to solve this properly: every case is fully synthetic, generated fresh each session, built around realistic clinical patterns rather than real records. Cases are built around real resistance patterns and current IDSA-aligned reasoning, so candidates are trained to think the way an antimicrobial stewardship round actually works.

The 6 elective categories cover patient–ventilator asynchrony, obstructive disease and dynamic hyperinflation, weaning and spontaneous breathing trials, capnography, oxygenation and refractory hypoxaemia, and a structured DOPES approach to ventilator alarms and the acutely deteriorating ventilated patient. This is one of the most visually rich generators, with genuine waveform and loop tracings built into the cases for candidates to interpret.

The 4 mandatory categories are critical appraisal of a landmark ICU randomised controlled trial, calculating and interpreting Number Needed to Treat/Harm, sensitivity and specificity from a diagnostic 2×2 table, and probability with confidence intervals. The 6 elective categories cover descriptive statistics, measures of spread, hypothesis testing and p-values, risk measures (relative risk and related ratios), sample size and statistical power, and correlation and regression. Every case is anchored to a real or realistic ICU trial or dataset, so candidates are practising the exact skill FCCCM and PCCCM exams test: reading a study and deciding whether it should change practice in an Indian ICU.

The fillable procedures logbook for Year 1 of the FCCCM fellowship. Covers the 7 mandatory procedures — endotracheal intubation, central venous cannulation, arterial line insertion, mechanical ventilation setup, non-invasive ventilation, chest tube insertion, and CPR leadership — plus 3 optional procedures (lumbar puncture, thoracentesis, paracentesis). Each procedure has its own log table: one row per case, capturing the indication, competency level reached (Observed / Assisted / Supervised / Independent), any complication, and free-text remarks — using the patient's hospital registration number only, never their name. A requirements summary on the cover page gives a running tally against the Year 1 minimums, and the mentor signs a declaration at the end of each procedure's table.

This is the one Procedures Log with an extra column: a Local Co-Signatory field for candidates on the PCCCM Remote Mentor Pathway, who need an on-site witness to co-sign each entry before their Remote Mentor's own endorsement. Candidates under direct on-site mentorship simply leave that column blank.
